This report from Medicare found that 13.5% of Medicare recipients who were hospitalized during October, 2008 suffered an adverse event which caused significant harm to the patient.  It was also determined that another 13.5% of Medicare recipients suffered an adverse event that resulted in only temporary harm to the patient.  Of note, physicians determined that 44% of all of the reported adverse events were preventable.
